MEDICAL RELEASE AND INDEMNITY AGREEMENT
PARENT PERMISSION FORM
THIS FORM IS VALID FOR ALL EVENTS OCCURRING IN CONJUNCTION WITH FAITHFUL FOUNDATIONS HUTCHINSON COUNTY

We, the undersigned, do hereby release, promise to save harmless and promise to indemnify FAITHFUL FOUNDATIONS HUTCHINSON COUNTY of Borger, Texas, (hereafter referred to as “FFHC”) for and from any liability whatsoever for any injury, damage, accident, misfortune, or death sustained by the undersigned or their minor children during the aforementioned activity.

We the undersigned further agree to hold the aforementioned FFHC harmless and to indemnify the same from any liability whatsoever resulting from any decision they, in their discretion, shall make including any accident, injury, damage, misfortune, or death which may occur to the undersigned or their minor children in connection with the activities of Faithful Foundations Hutchinson County.

This agreement shall be construed according to and governed by the laws of the State of Texas. All of the sentences, phrases, and provisions of this agreement are distinct and severable. If any clause, phrases, or sentence shall be deemed illegal or void or unforeseeable, it shall not affect the validity, legality or enforceability of any other clause or portion of this agreement.

We, the undersigned, have read this agreement in its entirety. This agreement was signed in Hutchinson County, Texas. By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.

Payment Instructions

Please note the following:

Registration fee is $25 per family- DUE IMMEDIATELY

Spring 2026 class fee is $25 per child (ages 3 and up) - DUE BY DEC 12th @ NOON TO HOLD YOUR CLASS SELECTIONS

Registration opens to the public for new members on December 12. If you haven't paid or made payment arrangements, we cannot guarantee your child's class selections.
